WebbSpecial Needs Assistance Program. This program provides you a one-time grant during a period of financial hardship. Requests are considered on a case-by-case basis. If you qualify, the grant may be paid directly to a vendor. The grant may help cover emergency expenses such as: WebbDual Eligible Special Needs Plans (D-SNPs) Dual Eligible Special Needs Plans (D-SNPs) enroll individuals who are entitled to both Medicare (title XVIII) and medical assistance from a state plan under Medicaid (title XIX). States cover some Medicare costs, depending on the state and the individual’s eligibility.
